    2. ICAMB Membership Registration Form
    3. E-Osh Certificate

UMT’s Postgrad Student Not Registered under ICAMB

Practical Industry Student

  • 8am – 5 pm / Working Days Only (Sunday – Thursday)
  • Every UMT’s registered student, Research Assistant or Practical Student (Industry)
  • To Access Laboratory After Office Hour/Weekend, You Must Fill out After Office Hour Form and submit it 3 days before the requested date.

  • Before allowed to use ICAMB Laboratory/Equipment, User MUST provide;

    1. Control of Substances Hazardous to Health Form (COSHH)
    2. ICAMB Membership Registration Form
    3. E-Osh Certificate

  • Not Access Card Given
  • To Access Laboratory, must be accompanied by a registered user or ICAMB Staff/Researcher

  • Before allowed to use ICAMB Laboratory/Equipment, User MUST provide;

    1. Control of Substances Hazardous to Health Form (COSHH)
    2. ICAMB Membership Registration Form
    3. E-Osh Certificate

Animal House User (For Animal House Only)

  • 24 Hours/ 7 Days (Animal House Only)

  • Before allowed to use Animal House, User MUST provide;

    1. Animal Ethics Form, Applied From RMO (Office of Research Management) 
    2. ICAMB Membership Registration Form
    3. Control of Substances Hazardous to Health Form (COSHH)
    4. Animal House Application Form
    5. E-Osh Certificate
