The applicant needs to identify the chemical to be stored and meets the officer in charge for any inquiry
Make an application through the Chemical Storage Application Form (UMT/ICAMB/LAB/007)
The applicant should check the specifications, quality and quantity from the supplier and obtain a copy of the SDS from the supplier as a reference.
Submit a copy of the Borang Laporan Terimaan Barang-Barang (PS-1) (UMT/ICAMB/LAB/008)(UMT-PS-1-IMB) attached with the delivery note (DO)/invoice, a copy of the PO and a copy of the SDS to the Officer.
Chemical will be stored in the chemical store
Applicants who wish to issue chemical, must fill in the Borang Permohonan Stok (UMT.PS-6-IMB)
The chemical stock report can be checked monthly which will be updated by the officer in the PTJ Monthly Stock Report (UMT.PS-4-IMB).
Chemical to be put into storage must be attached with a copy of SDS, a copy of purchase order (PO), a copy of delivery order and Goods Receipt Report Form (UMT/IMB/LAB/008 (UMT.PS-1-IMB)
Chemicals must be labeled (Supervisor’s name, and entry date), arranged and stored properly at the area provided.
Users need to fill out the ‘stock request form” UMT/IMB/LAB/010 form to take the chemicals for use in the laboratory.
Chemicals placed in the store area are single entry and maximum storage period is one year. Not more than one year; please renew your application form for extension.
Expired and unused chemicals should be removed from the storage area.
Chemicals that fail to be removed after the end of the storage period will be moved into the UMT laboratory consumable donation system.
It is the responsibility of the user to read the label and the Chemical Safety Data Sheet (SDS) which describes the dangers of a chemical to health and how it should be handled and stored safely.
